Paper form requests

Taxpayers who do not have access to print a form or file online can contact the SCDOR at forms@dor.sc.gov to request a paper copy. ​When emailing the SCDOR with your forms request, please provide the following information:

  • Your name
  • Your address
  • The name and number of the form you’re requesting

Request a copy of a tax return or tax account information/transcript​ on MyDORWAY

You must first have a MyDORWAY account to access y​our return history (sign up for a MyDORWAY account​ if you do not have one).​

To access a transcript on MyDORWAY:

  1. Log in to MyDORWAY and select the Accounts tab
  2. Scroll to the account you're looking for and click View Returns and Periods​ (for Withholding Tax accounts, click Payments, Returns, and Periods)
  3. Select​ the period you need a transcript for (Individual transcripts are available starting with tax year 2021)

Who can request copies? 

The SC4506, ​Request for Copy of Tax Return or Tax Account Information/Transcript​ must be signed and dated by the taxpayer for whom the return/information is sought:

  • Individuals
    If a taxpayer jointly filed a tax return, either spouse may submit the SC4506. Only one signature is required. Print your name on the SC4506 exactly as it appeared on your original return. If you changed your name, also print​ your current name. 
  • Corporations
    The SC4506 can be signed by:
    • An officer having legal authority to bind the corporation
    • Any person designated by the board of directors or other governing body 
    • Any officer or employee listed on a written request by any principal officer and attested to by the secretary or other officer 
    • Any shareholder of record owning 1% or more of the corporation’s outstanding stock, though they must submit documentation to support their right to receive the information. 
  • Partnerships
    Generally, the SC4506 can be submitted by any person who was a member of the partnership during the time period requested. 
  • Deceased or insolvent taxpayers and dissolved corporations
    Any trustee, guardian, executor, receiver, or administrator acting for the taxpayer or corporation may submit the SC4506. 
    • ​NOTE: Heirs of law, next of kin, and beneficiaries must be able to establish a material interest in the estate or trust. 

What documentation must be provided? 

For entities other than individuals, you must attach authorization documents. Examples include: 

  • A letter from the principal officer authorizing the employee of the corporation 
  • A letter of testamentary authorizing an individual to act for an estate 

Representatives
A representative can sign the SC4506 for the taxpayer only if they have been specifically designated as an authorized representative. A SC2848, Power of Attorney and Declaration of Representative​ showing this designation must be attached to the SC4506.

Commonly requested forms

When would the SCDOR deny a request for a paper copy of a return or form?

  • The SCDOR will not send a paper copy of returns that taxpayers are required to file electronically.
  • The SCDOR will not answer requests made by tax professionals or businesses that have access to a printed form or electronic file.
  • The SCDOR will not mail more than one copy of a form.

Need a form that is not administered by the SCDOR?

  • Federal forms, including the federal 1040, 1040SR, 1040EZ, W-2, and 1099, are available at irs.gov​. ​
  • Homestead Exemption forms are provided by the county auditor’s office where the real property is located. 
  • Unemployment forms are available at scdew.gov​.
